Student Registration

Listed below are the required forms that are needed to start the processing for a clinical rotation at Fairbanks Memorial Hospital/Denali Center in Fairbanks AK. Please consult a student rotation coordinator if you have any questions.

Required Forms:

• State of Alaska Background Check Form (Signature Required)

• Learner Information Page

• Banner Learning Center Coursework

• Student Badge and Parking Permit Form (Signature Required)

• Fax Cover Sheet

All forms must be complete or there could be a delay in processing your registration.

Returning Forms:

1. Please print all forms and fill out completely with signatures that are required. Fax forms to Interior AHEC at 907-457-4850 using the fax cover letter provided.

2. Submit a scanned image by using a .jpg, .pdf, .png or similar file. Once signatures have been preserved email forms to: joyce.hurt@bannerhealth.com
